We have an excellent opportunity for a conscientious and caring Social Worker in a fast-paced environment.
Responsibilities (including but not limited to):
Advocates for all residents' rights and preferences in their daily living.
Establishes strong relationships with families to support high quality care and goals.
Works closely within the interdisciplinary team to achieve organizational goals of the community.
Plays a key role in all aspects of the behavior management process.
Coordinates Discharge Planning process, which may include identification of barriers to discharge, implementing support services and/or educational opportunities for resident/resident representative Coordination of the PASRR Form Process and communication of targets with the local UMR Office.
Performs MDS 3.
0 interviews for Mood, Depression, and Cognitive Status based on requirements of the Resident Assessment Instrument (RAI) Manual Performs documentation of progress notes.
Creates and updates Social Services Care Plans Provides assistance with insurance appeals process, as applicable.
Requirements:
Bachelor's degree in any Human Services Related Field including but not limited to Sociology, gerontology, special education, rehab counseling and psychology Experience in Long Term Care/CCRC, a plus but not a must Willing to train a qualified candidate We are people, respected and appreciated, serving people, respected and appreciated.
